Indiana's rural health clinic network faces documented workforce shortages, with 62 of 92 counties designated as Health Professional Shortage Areas for behavioral health. The opioid-involved death rate in non-metro counties reached 28.4 per 100,000 in 2022, exceeding the state average by 31 percent.
Capacity constraints center on the limited number of waivered prescribers outside Indianapolis, Fort Wayne, and Evansville metro areas. Clinics in the 15 southernmost counties report average wait times of 47 days for medication-assisted treatment intake.
Applicants must provide letters of commitment from county health departments and existing Federally Qualified Health Centers confirming referral protocols. Standalone training proposals without established clinic partners will be rejected.
Infrastructure requirements include demonstrated access to Indiana's Prescription Drug Monitoring Program and ability to integrate data with the state's 14 regional recovery hubs. Budgets must show allocation for ongoing toxicology screening rather than one-time training events.
Funding favors organizations that commit to placing at least one full-time addiction specialist in clinics serving counties with fewer than 25,000 residents, with quarterly reporting on patient retention at 90 days post-intake.
