Indiana's transportation barriers for low-income patients are concentrated in the 28 counties classified as rural by the Indiana State Department of Health. These counties contain fewer than one primary care physician per 3,500 residents and lack fixed-route public transit outside limited city systems.
Compliance risks arise from the interaction between state Medicaid non-emergency medical transportation rules and the federal requirement that funded shuttles meet Americans with Disabilities Act vehicle standards. Organizations must maintain current Indiana Department of Transportation for-hire permits and drug-testing records for all drivers.
Risk mitigation documentation includes proof of $1 million commercial auto liability coverage and collision reporting procedures that align with Indiana Bureau of Motor Vehicles requirements. Any vehicle used for patient transport must pass annual inspection at an Indiana State Police certified facility.
Applications must include a county-by-county ridership projection based on Indiana Family and Social Services Administration claims data showing missed appointment rates. Letters of support are required from each county health department where service is proposed.
Funding is restricted to 501(c)(3) entities that already hold active contracts with Indiana Medicaid or a Medicaid managed care entity. Quarterly compliance reports must contain driver certification status, vehicle inspection certificates, and aggregate trip counts by county without patient identifiers.
