The state's opioid settlement allocations have prioritized substance use treatment over cardiac rehabilitation infrastructure, leaving limited capacity for combined physical and emotional support services.
Black residents in Marion and Lake counties and Amish communities in northern Indiana show distinct neuromuscular presentation patterns that standard clinic schedules do not accommodate. Shift workers in automotive and pharmaceutical manufacturing experience repeated cancellations of cardiac follow-up appointments due to mandatory overtime.
Integrative programs that combine prescribed exercise with behavioral health components have demonstrated reduced 90-day readmission rates in limited pilots at Eskenazi Health and IU Health. Scaling requires explicit linkage to county health departments that already operate chronic disease registries.
Funding supports hiring of licensed clinical social workers and exercise physiologists to operate weekly sessions at existing community centers rather than new construction. Outcome tracking uses the Indiana Network for Patient Care to measure changes in emergency department utilization for enrolled patients.
