These outcomes matter because Indiana's manufacturing and logistics workforce produced a cohort of retirees with elevated rates of diabetes and COPD, conditions that respond to coordinated community interventions. The Indiana State Department of Health tracks these metrics at the county level through its syndromic surveillance system.
Successful applicants must submit letters of commitment from at least two acute-care hospitals and one Area Agency on Aging designating shared data-use protocols compliant with Indiana's Health Information Exchange participation agreement. Coalitions must also identify a single county-level lead agency responsible for submitting quarterly encounter data to the state health department's existing chronic disease registry.
Implementation plans that rely solely on primary care referrals without hospital partnership receive lower scores. Budgets must allocate no more than 15 percent to general administrative costs, with the remainder tied to documented personnel or contracted service lines that directly generate the tracked encounter reductions.
