These counties cluster in the southern and western regions where manufacturing plant closures have reduced employer-sponsored insurance coverage. The Indiana Department of Health's 2022 burden report identifies gaps in preventive cardiology education reaching an estimated 310,000 adults below 200% of the federal poverty level.
Existing networks of federally qualified health centers and county health departments already serve these populations but lack updated cardiovascular risk stratification tools. Academic investigators at Indiana University and Purdue hold primary research capacity, yet most current grants concentrate on Indianapolis and Lafayette campuses.
The funding requires applicants to route resources through the Indiana Clinical and Translational Sciences Institute's regional hubs. Proposals must include county-level data demonstrating how heart health materials will reach manufacturing workers and former coal communities. Budgets must cover adaptation of existing Indiana University outreach curricula rather than creation of new generic content.
Readiness criteria include confirmation that at least two partner sites hold current CLIA waivers for point-of-care lipid testing. Projects must report enrollment numbers stratified by insurance status to verify reach into the uninsured manufacturing workforce.
