Indiana's Digital Infrastructure Barriers in Sarcoma Research Participation
Indiana reports sarcoma incidence rates consistent with national averages, yet electronic health record fragmentation across 120 independent hospital systems limits cohort assembly. The largest integrated system, Indiana University Health, covers only 38 percent of the state's oncology encounters. This fragmentation creates a barrier to uniform data capture for multi-site genetic studies.
The funding mechanism supports development of a lightweight, HIPAA-compliant mobile platform that pulls discrete data elements from participating institutions via HL7 FHIR APIs. Priority is given to platforms that operate on low-bandwidth connections common in rural southern Indiana counties. Proposals must document existing relationships with at least three health systems outside Marion County and provide letters confirming IT security review completion.
Applications require a data dictionary specifying which variables will be standardized across sites, including tumor grade, treatment dates, and self-reported ancestry. The state anchor of note is Indiana's high proportion of manufacturing and agricultural workers whose occupational exposures may interact with genetic risk; platforms must therefore include structured fields for job history coded to NAICS categories. Budgets may request up to $120,000 for software development and cybersecurity audits but must allocate at least 20 percent of personnel time to training community oncology nurses on data entry.
Projects are evaluated on whether the digital tool reduces time from consent to data availability to under 48 hours. Quarterly metrics must include patient retention rates at 6 and 12 months and the percentage of records successfully linked to Indiana's cancer registry.
